Admittedly my husband is balinese but even before we married it was the same.
1) Your sponsor letter
2)Copy of sponsor's KTP front & back.
3)2 photos

I have never had to show bank details of either my own or my husbands.
The visa is issued for 60 days. You have to use it within 3 months.
On arrival in Bali make sure Passport control stamp your passort for 60 days too.(stamped mine for 30 last time-I didn't notice till time to extend & had to back to Bali, from Lombok to get it sorted)
A week or more before the end date you go to immigration to extend for another month. The cost is Rp200,000 or Rp250,000, I forget. Definitely NOT Rp350,000!
You extend on a monthly basis till the 6 months are up.
On one extension month 4/5 you get finger printed.
On the following extension your sponsor will have to go to the "justice" you do not have to accompany the sponsor & best you dont as the justice will ask a "fee" from you!
A good way to avoid paying extras is to ask for a receipt for everything you pay.
Each extension actually involves a few trips to immigration. Once to apply & they will get you back a few times to pick it up before it is actually ready!
Its well worth the hassle though.
