I can highly recommend Bali International Diving Professionals. Based in Sanur. I did an Open Water Referral course - did my theory/pool stuff in the UK and my qualifying dives with them in 2002. They were great. They provide 1-2-1 instruction and really look after you. I'll be diving with them again in September, taking a friend along to do her OW dives too. As they only dive with very small groups, they will probably take just the two of you if you want. Just about all of Bali's dive sites are within a day's travelling distance, although it is exhausting if you go to the furthest sites. But they provide all of the transport and your lunches/water.
The only thing I would say is once you've completed your course do a couple more fun dives to see the stuff you might have missed on your qualifying dives. I had the misfortune to miss a manta ray whilst practicing underwater navigation, and I didn't really have chance to have a good look at the Liberty wreck as I practiced removing and replacing my mask!!
