I think that you might have to look at the 5* hotels such as the Hilton in Nusa Dua where I remember the huge bathroom with a separate shower although there was a raised lip at the entrance, about 2 inches high I think.
An alternative might be a place with an outside shower which simply means that part of the roof is open so that the room dries out and is less liable to grow mould and therefore can do without extractor fans.
I would have thought that this is the sort of information that your travel agent should be able to provide with some certainty. If not I suggest that you get in touch with Putu Les or David at Golden Bali Travel in Adelaide. I'm sure he would know.
