I'd definitely boil the bottled water. It's seriously cheap, so use it for everything, like brushing teeth etc. I would even try to not swallow any water while showering etc. From memory, I think I was paying around Rp10,000 for a 4L bottle and I'd use that to top up smaller bottles and take them out with me for the day, as well as for brushing my teeth.
As far as warungs, I guess it depends on each one. Some people buy from the mobile food sellers and have no problems - others do so and get ill. I guess it depends what you buy and from where. Of course some people get sick at more expensive restaurants too, so price is no guarantee of safety.
I tend to stick to cheaper to middle priced places for a lot of my meals when in Bali. Small warungs have been fine - provided they seem like they have a high turnover of food.
I guess you just have to use your own judgement when checking places out.
