We did do some snorkeling off of Trawangan and it was fine. I think any of the glass-bottom boats from any of the 3 islands will take you to all three islands and stop in Gili Meno or Air for lunch.
We did not take any malaria profelactics. You'd have to be really unlucky to get malaria in the Gilis. However, we did meet a Danish girl who had gotten a mild form of Dengue Fever in Gili Trawangan. She went back to Sengigi (where I met her) for good and cheap treatment.
I've had malaria 3 times myself; once in Uganda, once in Cameroon, and once in Ghana. You might get it if you're traveling in South America for a year. You will very likely get it if you spend a year traveling in Africa. If you're spending 2 months or less - mostly in Bali/Lombok-, I wouldn't take any precautions other than making sure I had a good fan or mosquito net in Lombok/Gilis. Remember sunset is the most dangerous time; so cover up, use some "Off" at night, and spray your room with "Hit" when you go out.
Drowning or heat stroke are greater dangers than malaria for most people in my honest opinion.
