It was either Mirra or Mirror Boxing. If you jump into a taxi they will know where it is. It isn't the nicest place but you get an awsome work out.
Not sure of the name of the company and yes it was for the three activities. Just ask at your hotel or you can go to Coral Reef Cafe and see Eddie and he'll plan it for you. He always books our tours for us and he only recieves about 5% commission but what I've found he gets some of the best prices. Other people at our hotel were paying US$25 for jet skiing alone. Plus Eddie is a crack up and he'll even take you for a tour.
