When having a massage on the beach or in the massage pavillions bathers are left on. They may pull your shoulder straps down for better access, or if wearing a bikini they will ask you if you want to remove the top. You can tell them that you want to leave it on also.
In the day spas you generally remove all clothing, they sometimes provide disposable undies. Once again if you are shy about displaying all you could ask to leave on whatever. Some places cover you with a towel, and some don't. Just let them know what you prefer beforehand and they will respect your wishes.
