Basically they identify themselves and the person they're sponsoring (name, age, address, occupation, copy of passport) and their "relationship" to the one being sponsored. "Friend/spouse/fiancee" are common relationships. A line or two about how he/she wants to learn more about Bali, its culture and the sponsor's family than he/she would be able to do in a normal tourist visit. You could also point out that the person has no interest or intention to try and work in Bali. That's about it.
