A signed letter from your Indonesian 'sponsor' and a copy of his/her KTP (identity card); plus a couple of passport sized photos and a copy of your ticket out of Indonesia.

Incidentally, I just got my latest SB in Perth the same day (just had to pay them AUD 125). Oh, and you should be aware that the initial visa that goes in your passport says 60 days. That's right. One week before your 60 days are up, you have to report to the Immigration office to extend it for a further month (and pay, of course - how much sort of depends on whom you're dealing with; can be Rp 200,000 or more). And you have to go through this process every month until you're finished.

Having said that, I also know that it's possible to extend more than one month at a time, again, depending on whom you're dealing with. I've been living in Bali more than 5 years now on this type of visa.
