I don't know the name of the shops, but the two in Kuta is opposite the Bali Dynasty Hotel, a little to the north. The shops in Legian is in the same street as the Indo National restaurant. It's a one way street (used to be called Garlic Ave, but has another name now), the shops are in a little avenue off this street, on the south side.
That's the best I can do. But if you wear a real surf shirt on the street, you'll quickly attract hawkers trying to sell you the real stuff.
