The Rama Beach Resort and Spa (previously named Rama Beach Cottages) in Tuban.
It is 3 star, but has 5 star pool area. We have 2 kids who just loved it. We booked 2 interconnecting rooms (because it's not 5 star, was affordable to do this).
Took the PS2 with chip installed. The kids were kept occupied with all the DVD's and PS2s they were able to buy so inexpensively ($1.50AUS) and watched them in their own room as time out.
Hotel also has pool table and table tennis table. No kids club. Staff are fabulous. Because wse stayed greater than 5 days, we were upgraded to Deluxe room and had free breakfast for the duration of our stay. The rooms actually allow 2 extra beds in the rooms free of charge. I suggest you book one interconnecting room for yourselves (king size bed) and another interconnecting with 2 singles and add 2 mattresses free of charge.
From Rama Beach you can take a beautiful walk along beach to main Kuta. The hotel is just far enough away to not be in the hustle bustle of it all. Very close to Bali Hai, Patra Jasa and next to Green Garden Hotel (3hr spa here for $37AUS!) Otherwise catch a cab into kuta for about $1.20Aus, takes 2mins depending on roadwork/traffic.
Also, the hotel is across the road from the beach and diagonally oppposite the Pantai Restaurant (arguably the best restaurant in Bali - forget Jimbaran Bay....the Pantai in on the beach, you watch the same sunset, don't have to travel or pay the Jimbaren prices).
