Your hotel in Nusa will be able to help you. We emailed them with many questions prior to going. They had a car and driver awaiting us at the airport. I can't recall the exact price for a taxi to Kuta from the hotel but it was minimal.
We were SO glad we stayed in Nusa dua and only visited Kuta.
