You shouldnt have any trouble getting vegetarian meals at all, there are so many different sorts of cuisines over there that I'm sure its not going to be a problem. The balinese are used to requests for different sorts of meals as diets vary considerably on the island due to the different religions that are to be found there and of course the tourist who come from all over the world. As for smoking, its pretty much everywhere and people light up without compunction in almost every restaurant, I didnt notice any dedicated "non smoking" sections at all, in the restaurants we ate in - maybe in the more expensive eateries they may have different sections. Smoking in Bali seems to be a right diligently exercised by the male populous and the tourists seemed to be at it a fair bit as well. My advice would be to maybe see if you can get a table somewhere near the outside of the restaurant, other than that, scan carefully at the tables before taking a seat and hopefully you will be surrounded by other non smokers, good luck. Oh, the restaurants are mainly open plan affairs so the smoke does seem to clear a lot quicker, I didnt really notice it all that much and I am a non smoker also.
