I can't answer all of your questions, but I can help you with a few.
If you need cash when you get there, just use the ATM in the airport
for starters.
Getting through customs is very easy, just a few minutes at most for
both of you.
I would recommend getting hepatitis A, tetanus and flu shots before
you go. We did, and stayed very healthy. Also pick up some waterless
hand cleaner (Purell, if you can get it) and use it often.
There are lots of transport vans outside the airport, you may want
to throw your bike in one and get the driver to take you to a lossman,
and point out a service station on the way. Or book a lossman your first night from home and get them to send a vehicle to meet you.
Hair plaiting can be done on the beach anywhere around Kuta.
Remember the roads there are very narrow and everyone drives like
maniacs. Personally, I wouldn't bike on the busier roads, though
I am not familiar with the Lonely Planet biking itinerary. Sounds
like a great trip, looking forward to hearing about your travels
when you get back.
