Hotel in Kuta or near $40$50 suggestion?

Please give advise where to stay, I will be coming to Bali in May.
I want to stay where they have the cleanest faclities, and a nice workout room. Also the hotel should have a couple of restaurants, including Indonesian, if that is possible.
