A fair price is really what you are willing to pay without thinking that you got ripped off. We had a big 4x4 and it worked out to 10 dollars Australian a day for 4 days. We went to an actual petrol station and paid 16000 rupiah to fill the tank. 500 kms of driving. I didn't think that was bad. The petrol wasn't out of the pump, but instead a 45 gallon drum and hand pump. Pretty dodgy but better than the bottles on the side of the road that you were talking about. In response to your last question, Finding your way out of Kuta is slow and stressful, but I feel worth it to have the freedom of going wherever you want. We did however get lost a couple of times because the roads are not well signed!! We bought a map and showed it to people when we were lost, yet they had no clue where they were on teh map. This is a long one, phew!! One other tip watch out for the police, we heard rumours that they will set up road blocks and ask you for money. Although we went through two such roadblocks without incident. Always remember to WATCH OUT FOR MOPEDS!!
