Returned a few days ago. Next time I might get a cash card with a Mastercard logo. One major bank's ATMs accept Mastercard but not Visa. It seems to be the bank in the towns and areas I would try (or maybe there are several banks like this, I didn't pay attention to the names). I did find that I could go into these banks with my passport and Visa logoed ATM card and get as much cash as I requested (never asked for more than 1,000,000 rupiah so don't know how high I could have gone) for a fee of 15,000-20,000, paid direct at the time of transaction. The banks were always efficient and friendly with this service.

Do take more than one credit card/ATM card if you are depending on it for cash or purchases. Met a woman over there whose ATM card that was fine at home would not work (fortunately she was with family and borrowed a card that did) and another woman whose card was cancelled by the bank as stolen after she inadvertently tried to use the wrong card's code with it at an ATM. Plastic technology is good, but not perfect.
