Go to Tendi Tailor at Poppies Lane 2 in Kuta beach (take a metered taxi).
The made my suit and it's very good. I don't no of materials, my wife made the selection and she says it's high class.
If you give them 3-4 days it's fair. They made mine in 2 days over the weekend. You don't need to bring anything, the take all the meassurements they need.
The have kind of fixed prices but you can always get better prices if you by more, like 2 jackets.
I paid $US 100 for one complete suit with west, 2 shirts and one extra jacket, a very good price.
