Lots of great posts and, while I don't agree with all of them, there are many I am sure are shared by almost everyone who is fortunate enought to visit Bali.
These are my "worsts" in no particular order:
the hawkers at the overlook at Mt. Batur (the first time in years I'd been ready to pummel someone; had he grappled my wife one more time it would have been very ugly. as it turned out we reached a "gentleman's agreement")
the pollution in and near the developed areas in the south
the local trash dump located just outside of most small towns
the fact that it is half a world away (my problem not theirs)
the poverty (hard to know what to do about this aside from paying reasonable, haggled prices for things, trying to employ Balinese/locals as much as possible, and paying my taxes to support the IMF/UN/WHO when we pay our dues.)
Anyway, that's about it. Most of the other stuff people mention didn't bother me much or were avoided by knowing beforehand what to expect.
