I drank watermelon or lemon juice drinks my whole time in Bali. I did get the odd mild case of the runs (Immodium fixes that up quick), but I don't know the juice was responsible, and I think this is inevitable in Bali. Sometimes the lemon drinks had crushed ice in them.
I think it all depends on the bar - bear in mind that a lot of the places make ice from bottled water - I got more and more trusting as time went by and was never really sick.
I think the warnings against fruit are bases on that they may have been washed in bad water, or may include rind with bacteria on it. Again I started out with watermelon & lemon because I figured that you didn't puree the rind in either case.
Warning though: Vodka and Orange probably means bottled orange juice as fresh orange is very uncommon there. Their bottled orange is rather peculiar - yellow and watery and not tasting much like orange at all.
