some of your questions I can answer as I spent two very relaxing vacations in Bali. Its a real recharge for body and mind.
1. I don't dive but know many who do. Although its not the Great Barrier Reef or even Manado, many visitors really like it. The best spots are near Menjanagan, an island in the North-West, and along the East coast.
2. In my opinion the perfect place are the "Beach Resort Deluxe Lanais" located right on the beach a few miles north of Kuta. You have lots of space (incl. open living/dining area), total privacy, telephone and satellite TV if you want it, friendly service, and its located away from the mass tourists but close to many restaurants, discos, shops etc whenever you feel like it. 3. Take the 3-day cruise or even the 4-day cruise. This includes Lombok, and I think one day is enough for this in my opinion over-rated island. Book the cheapest cabin -- you'll only use it to sleep.
4. I would rent a small jeep for the whole stay. For excursions you should rent a driver which is also a kind of guide. Otherwise you can drive yourself (bring an international license).
5. You get great and cheap seafood at the open beachfront restaurants at Jimbaran. There are also a few places in Kuta. The Khin Kao is a recommendable Thai restaurant in Seminyak. Look at the recommendations on the "Balivillas" site.
6. Afternoons you can go to Made's Warung in Kuta. Early evenings to Poco Loco (Mexican restaurant) in Legian. Later its Cafe Luna, Goa, and the Jaya Pub (life misic) in Seminyak, and in the early hours either Gado Gado or Double Six Disco on the beach in Seminyak. Again, all these places are not far from the "Beach Resort Deluxe Lanais" -- you even could walk there along the beach.
